A 90-year-old male was admitted to the hospital for right leg swelling. 20 years prior to his admission he had an open repair of an infra-renal abdominal aneurysm with an aorto-biiliac bypass and ligation of the ostium of both internal iliac arteries. CT scan showed a massive right internal iliac artery aneurysm of 18 cm in diameter (Figures 1A-1C) with a patent superior gluteal artery. His aneurysm was first embolized through a transabdominal percutaneous approach and later on a surgical resection was carried through an infra-ombilical midline incision with aneurysmorrhaphy and ligation of the superior gluteal artery (Figure 1D).
